California Healthy Kids Surveys
Student and Parent input is gathered during the yearly Local Control Accountability Plan renewal. Below are the surveys students in 3rd-8th grade will take. Parents are asked to give permission for students in 3rd-6th grade to complete the survey. Parents of students in 7th and 8th grade can opt their child out of the survey. Permission and opt-out is sent out by the school site before the survey is administered.
