In addition to the enrollment packet forms listed above all students must be registered by a parent/guardian and provide the documents below:
California schools are required to check immunization records for all new student admissions at TK/Kindergarten through 12th grade and all students advancing to 7th grade before entry.For more information go to "Shots for School", a California website about immunizations required for school entry http://www.shotsforschool.org
Verification of Residency Children of legal age may attend schools in the district after providing proof of residency. Proof of residency is required to be updated annually verification of residency.
Birth Certificate, baptismal certificate, passport, affidavit, or other alternative forms of documentation to verify age.
Proof of immunizations against polio, measles, mumps, and rubella (MMR), DPT (Diphtheria, Pertussis, and Tetanus), hepatitis B, varicella (chickenpox).
Kindergarten Students (new to the district or attendance area)
For more information about immunization requirements and availability of vaccinations, contact your home school nurse.
Copy of a birth certificate, baptismal certificate, passport, affidavit, or other alternative forms of documentation to verify age.
Polio: Four doses must be completed
DPT (Diphtheria, Pertussis and Tetanus): Five doses must be completed.
MMR (Measles, Mumps, Rubella): Two doses must be completed on or after your child's first birthday.
Hepatitis B: Three doses of Hepathitis B vaccine must be completed.
Varicella: One dose given on or after the child's first birthday, OR health care provider documentation of Varicella disease or immunity. Students are encouraged to have the Varicella booster prior to school enrollment.
Please note: Effective July 1, 2019 students need two (rather than one) doses of varicella (chicken pox) vaccine.
